A 68‑year‑old male patient with small‑cell lung cancer received six cycles of combination therapy with adebrelimab plus etoposide and carboplatin. Following the final administration of the immune-chemotherapy regimen, the patient developed extensive erythema, desquamation and pruritus on the head, face, neck, chest, hands and extremities, accompanied by oral ulceration, and the patient was diagnosed with grade Ⅲ delayed‑onset rash. After infection and other predisposing factors were ruled out, the rash was considered to be adebrelimab‑induced. Symptoms improved after treatment with high‑dose glucocorticoids and antihistamines. The rash relapsed 11 days after tapering of glucocorticoids, and the patient was readmitted. The aforementioned therapeutic regimen was re‑administered, resulting in rash improvement. Glucocorticoid‑induced diabetes mellitus occurred during the treatment course, for which antidiabetic therapy was initiated, and adebrelimab was permanently discontinued. Assessment via the Naranjo's Probability Scale indicated a probable causal relationship between the rash and adebrelimab. This case highlights that prolonged monitoring of cutaneous adverse reactions is warranted during adebrelimab treatment. High‑dose glucocorticoids should be promptly initiated for grade Ⅲ rash, with intensified glucocorticoid dose titration and blood glucose monitoring to reduce the risk of immune-related skin adverse reactions and glucocorticoid-related complications.
