Objective To systematically evaluate the efficacy, safety and of Hudi Enteric-coated Capsules for ulcerative colitis (UC) , and to conduct pharmacoeconomic research, so as to provide guidance for rational drug use and provide evidence for making relevant healthcare decisions.

Methods Randomized controlled trials (RCTs) regarding Hudi enteric-coated capsules for UC were systematically retrieved from the Cochrane Library, PubMed, Embase, Web of Science, SpringerLink, CNKI, WanFang Data and VIP databases. The retrieval period was from database establishment to March 31, 2026. Meta-analysis was performed using RevMan 5.2. A short-term decision-tree model was constructed using the pooled total effective rate as the effectiveness parameter, and the incremental cost-effectiveness ratio (ICER) was used for economic evaluation, and one-way and sensitivity analyses were performed.

Results A total of 8 RCTs involving 1,023 patients were included. Hudi enteric-coated capsules could improve clinical efficacy (OR=5.11, 95%CI 3.41 to 7.65, P < 0.001). The total effective rates of the experimental and control groups were 92.72% and 74.36%, respectively. The incremental drug cost was 1,638.00 CNY, and the ICER was 8,922.24 CNY per additional effective case, which was lower than the willingness-to-pay (WTP) reference threshold of 43,377 CNY, showing superior cost-effectiveness. Probabilistic sensitivity analysis indicated that the probability of cost-effectiveness was 100.0%.

Conclusion Current evidence suggests that Hudi enteric-coated capsules may improve clinical efficacy and show favorable short-term cost-effectiveness in UC treatment; however, high-quality RCTs and real-world data are still needed for further verification.
