Construction and practice of individualized medication guidance for cardiovascular chronic disease patients based on informatization

Abstract

Objective  To explore the methods and significance of individualized medication guidance for cardiovascular chronic disease patients under the pharmaceutical informationization construction, and provide references for pharmacists to carry out medication education.

Methods   An independent medication guidance system was designed, and a medication guidance database was established by maintaining drug dictionary data and drug image information in the system. The patients’ visit information and prescription information were corresponded to the medication guidance database, and illustrated medication guidance content was provided for cardiovascular chronic disease patients, so as to achieve full coverage of individualized medication guidance for all patients in the hospital.

Results  2 286 kinds of drug dictionary data, 498 drug atlas data, and 570 cases of medication precautions were maintained in this system. The medication guidance database based on the information system can meet the needs of individualized medication guidance for patients. After implementation of the system, all discharged patients in the hospital could obtain a paper version of the medication guidance form. Outpatient patients and Internet consultation patients could receive medication guidance push through the hospital's WeChat public account, which reduced patients' waiting time for medication pickup and improved the efficiency of medication education. A total of 146 000 inpatients were provided with medication guidance, and a total of 123 000 outpatients and Internet patients were pushed with medication guidance content.

Conclusion  The application of the information system to achieve visual, multi-scene application of individualized medication guidance integrating drug verification and medication guidance improves the efficiency of pharmacists' medication instructions, ensures the safety of patients' medication, and provides references for innovating pharmaceutical service models and improving patients' medical experience.
